Requests for Indications & Expanded History

As your radiology provider, we want to deliver the most complete Inside Story on your patients with comprehensive reports.To do that, we're asking for your assistance in gathering additional details to ensure that our Radiologists are able to completely answer any questions your providers may have. Having this information up front may help reduce addendum requests for additional reviews and comments on exams.

When completing the online order entry for an exam, the indication provided within the physician reason section is what populates within the report and should be specific. However, the order notes section can and should be used to provide any additional information to help guide the reading Radiologist, such as a request to "rule-out" or address a "possible" or "suspected" condition. 

For example, the physician reason provided may be "pain and swelling after traumatic injury" with the order notes section adding a request to "rule-out fracture".

In a situation where a Technologist gathers additional information from the patient directly, that additional information can be used for coding *if* it has been documented in the patient's medical record.
