Updated LDCT Worksheet

We've updated our LDCT worksheet to include "Screening for Lung Cancer" as the indication for the exam. When performing a Low-Dose CT of the Chest for a lung cancer screening, the patient should be asymptomatic. We made this update to the LDCT worksheet to help with that reminder as other indications could result in denied claims.
